Rapid diagnostic tests lymhatic filariasis

There are two types of lymhatic filariasis(LF) namely brugian and bancroftian LF, the latter comprised 90% of the global disease burden.  The patented BmR1 recombinant protein was discovered via a targeted immunoscreening of a Brugia malayi cDNA library.  It was then used for development of a lateral flow cassette test called Brugia Rapid for detection of B. malayi and B. timori and has been licensed to Reszon Diagnostics International Sdn. Bhd.  The test had undergone extensive multicenter evaluations/validations in the laboratory and in the field, as well as under operational conditions.  Brugia Rapid has been recognized by the World Health Organization (WHO) for use as a diagnostic tool in brugian LF countries as part of the Global Programme for Elimination of Lymphatic Countries (GPELF). PanLF Rapid, also commercialized by Reszon, is an extension of Brugia Rapid, and contains an additional infection marker called BmSXP that is sensitive for detection of bancroftian filariasis caused by Wuchereria bancrofti. It is meant for use in brugian LF countries that has some areas of possible co-infection with bancroftian filariasis. The Ministry of Health Malaysia is using PanLF in its LF elimination program. BLF Rapid is a prototype rapid test kit for highly sensitive detection of bancroftian filariasis. It has been evaluated in USA (at CDC Atlanta and University of Washington, St Louis),  India and Malaysia with excellent results.

Rapid disgnostic tests for amoebiasis

Amoebiasis is caused by the protozoan parasite, Entamoeba histolytica that causes intestinal and extraintestinal amoebiasis. Due to the lack of a rapid test for the latter, we embarked on the analysis of excretory-secretory antigens (ESA) of E. histolytica which led to the discovery of pyruvate phosphate dikinase (PPDK) as a good diagnostic marker for amoebic liver abscess, and a patent has been granted for this finding. The recombinant form of PPDK was produced and a lateral flow rapid IgG4 test was developed which is called XEh Rapid.  The prototype test has undergone multicenter national and international evaluations with positive outcomes.  In addition, we have developed a proof of concept rapid antigen detection test for intestinal amoebiasis using antibodies to rPPDK and ESA. Although the test showed better sensitivity compared to the major antigen detection ELISA in the market, further work will be undertaken to increase the sensitivity of our test, and it will be followed by multicenter evaluations.

Rapid diagnostic test for other parasitic diseases

We are also working on developing and refining several other lateral flow rapid tests for parasitic infections.  Tc Rapid is a prototype rapid test for detection of toxocariasis, and it uses several Toxocara canis recombinant proteins, one of them has been patented. Since T. cati has been implicated in some cases of toxocariasis, especially in certain human populations where cats are more favoured than dogs, we are now working on improving Tc Rapid by incorporating a T.cati recombinant protein.

Another recombinant antigen-based lateral flow rapid test, Strongy Rapid, has been developed to detect strongyloidiasis and is now at an advanced stage of development.  Due to the phenomenon of autoinfection, this parasite can survive for many years in the body. Thus immunosuppressed patients who are living or used to live in endemic areas should be tested for this infection to prevent potentially fatal outcomes due to exacerbation of the infection. 

We have also developed a lateral flow rapid test (Hyd Rapid) using a unique recombinant AgB protein for specific IgG4 detection of hydatid disease. An antigen detection test for hydatid disease is also being developed using antibodies to various forms of Echinococcus granulosus antigens. In addition, an early prototype rapid test for IgM detection of toxoplasmosis has been developed using two recombinant proteins, one of them has been filed for patent.
